Mechanical complications following acute myocardial infarction, such as ventricular septal rupture, necessitate urgent management at tertiary centers with specialized surgical expertise. While surgical repair remains the definitive treatment, current 2025 ACS guidelines emphasize the role of short-term mechanical circulatory support, including intra-aortic balloon pumps, to achieve hemodynamic stabilization. This bridging strategy facilitates end-organ recovery and allows for infarct tissue maturation, which significantly improves surgical outcomes compared to immediate intervention. Although percutaneous closure techniques are evolving, they are currently reserved for patients at prohibitive surgical risk. Dr. Venu Menon, Section Head of Clinical Cardiology at the Cleveland Clinic, underscores that even stable patients face unpredictable hemodynamic deterioration, mandating immediate transfer to facilities equipped with multidisciplinary teams capable of managing these high-risk, complex clinical scenarios.
