Bad Ideas, Good Outcomes, and Thin Melanomas

Podcast cover

Derms on Drugs

Thin melanomas, despite their low thickness, present significant recurrence risks when characterized by high mitotic rates, tumor-infiltrating lymphocytes, or the absence of a radial growth phase. Clinical management of Hidradenitis Suppurativa (HS) may benefit from adding oral oxybutynin to adalimumab, as sweat reduction potentially alters the skin microbiome to mitigate flares. Dupilumab-associated facial erythema frequently stems from underlying Malassezia sensitivity, which responds effectively to itraconazole therapy. Genetic testing for familial melanoma genes is most appropriate for patients with multiple primary melanomas or invasive disease diagnosed before age 40. Furthermore, managing diaper dermatitis in infants requires frequent diaper changes—ideally every two hours—combined with brief periods of open-air exposure to accelerate healing. These clinical insights emphasize the importance of identifying specific patient profiles and risk factors to optimize treatment outcomes in dermatology.
