Tibial bone stress injuries represent the most common site of bone stress in runners, affecting up to 50% of male and 35% of female athletes. These injuries exist on a clinical continuum, ranging from manageable stress reactions to severe fractures. A critical diagnostic distinction exists between Medial Tibial Stress Syndrome (MTSS) and true bone stress injuries: if systematic palpation reveals soreness across a region greater than 10 centimeters, the condition is likely MTSS, which often allows for continued activity. Conversely, localized pain under 10 centimeters necessitates imaging and immediate deloading. Muscle force, particularly from the calf, significantly influences internal tibial loads, which can reach ten times body weight. Reduced calf girth correlates with a fourfold increase in injury risk, highlighting the necessity of maintaining muscle capacity as a primary strategy for both injury prevention and rehabilitation in endurance athletes.
