Parapneumonic effusions and empyema represent a spectrum of pleural infection requiring precise diagnostic and therapeutic approaches. Distinguishing between uncomplicated effusions, complicated parapneumonic effusions, and frank empyema relies on pleural fluid analysis—specifically pH, glucose, and LDH levels—alongside radiographic evidence of loculations. Ultrasound serves as the bedside gold standard for identifying these features, while CT scans provide essential anatomical context. Management prioritizes timely drainage via small-bore pigtail catheters, often supplemented by intrapleural enzymatic therapy using TPA and DNase to address fibrinopurulent debris. When medical management fails, early surgical consultation for video-assisted thoracoscopic surgery (VATS) is critical to prevent progression to chronic, organized disease. Diagnostic yield improves significantly by utilizing blood culture bottles for fluid samples and applying the three-test rule to differentiate exudates from transudates, ensuring targeted antibiotic and procedural interventions.
