Ep. 160: The Prehospital Trauma Compendium - Prehospital Blood, Vasopressors, TXA, and Traumatic Cardiac Arrest
Prehospital Emergency Care Podcast
The Prehospital Trauma Compendium provides updated evidence-based guidelines for managing traumatic injuries in the prehospital setting. Whole blood is now the preferred first-line treatment for hemorrhagic shock, necessitating regional collaboration to ensure availability and minimize wastage. Regarding vasopressors, current literature lacks support for their routine use in hemorrhagic shock, with some evidence suggesting potential harm, while their role in spinal cord and brain injuries remains unproven. Tranexamic acid (TXA) serves as a secondary adjunct to primary hemorrhage control and blood resuscitation, with early administration within three hours being vital. For traumatic out-of-hospital circulatory collapse, resuscitation efforts must prioritize reversible causes like hemorrhage control and chest decompression over routine CPR or advanced airway interventions, which often delay definitive care. These recommendations emphasize a shift toward targeted, high-impact interventions and the necessity of system-wide coordination between EMS and trauma centers.
