Mechanical ventilation requires a foundational understanding of breath types and delivery methods to manage patients effectively in the ICU. Breaths are categorized as controlled, where the ventilator manages all timing and force; assisted, where the patient triggers the breath but the ventilator completes it; or spontaneous, where the patient performs the work with varying levels of support. Delivery methods are split between volume-controlled, which ensures a set tidal volume, and pressure-controlled, which delivers a set pressure requiring monitoring of resulting tidal volumes. These mechanics directly correlate with lung compliance; a decrease in compliance necessitates higher pressures to maintain volume or results in lower volumes at constant pressure. Mastering these concepts provides a framework for navigating various ventilator modes and clinical scenarios, moving beyond rote memorization of settings toward a physiological understanding of respiratory support.
