REBEL CAST – RENOVATE Trial: HFNC vs BPAP in Acute Respiratory Failure

The RENOVATE trial evaluates whether high-flow nasal cannula (HFNC) serves as a viable alternative to non-invasive ventilation (NIV) for patients with acute respiratory failure. Utilizing a Bayesian adaptive model across 33 Brazilian hospitals, the study demonstrates that HFNC is non-inferior to NIV regarding the primary outcome of intubation or death within seven days. While both modalities show comparable results in most subgroups, NIV remains superior for immunocompromised patients, where HFNC proved futile. Notably, HFNC offers significant advantages in patient comfort and reduced sedation requirements, potentially altering clinical practice for conditions like acute cardiogenic pulmonary edema. Despite limitations regarding non-standardized sedation protocols and small subgroup sizes, the findings support integrating HFNC as a primary or secondary treatment option, particularly when patients struggle to tolerate traditional mask-based ventilation.
