If you say something off the top of your head, you say it without thinking about it much or without trying to check it or find out more facts about it.

如果你随口说出某件事（不假思索地说），意思就是你没有过多思考，

也没有尝试核实或查找更多相关事实。

翻译成中文有“不假思索、脱口而出；凭直觉（无准备情况下）”。

用于当某人没有提前准备，临时想到答案、观点或给出回应时。

例如：

I can't remember off the top of my head the date of the wedding.

我一时想不起来婚礼的日期了（凭记忆记不清了）。

OK, off the top of your head, do you know the capital of South Korea?

好啦，凭第一反应（不假思索地说），你知道韩国的首都是哪里吗？

You either have to kind of free ball it and just be, you know, Off the top of your head and just talk about how you're feeling or you have to have really good jokes.
