The Resus Room

Effective thermal management in trauma care requires the prompt removal of wet clothing, which significantly accelerates re-warming rates compared to leaving wet layers on. For patients with isolated limb fractures, a combination of intravenous ibuprofen and paracetamol achieves greater pain reduction at one hour than intravenous morphine, although morphine provides faster initial relief. In cases of severe traumatic brain injury, maintaining end-tidal CO2 levels between 35 and 45 mmHg is critical, as values below 35 mmHg show a strong association with increased mortality. These findings emphasize the importance of tailoring analgesia and ventilation strategies to specific clinical contexts rather than relying on standard protocols. Clinicians should prioritize rapid drying of patients in cold environments and avoid hypocapnia during pre-hospital airway management to optimize patient outcomes.
