Pleural effusions represent fluid accumulation in the pleural cavity, signaling underlying local or systemic pathology. Effective clinical evaluation begins with assessing patient stability, followed by identifying symptoms like shortness of breath or pleuritic chest pain. Physical examination findings, such as reduced breath sounds and dullness on percussion, necessitate diagnostic thoracentesis for effusions exceeding one centimeter. Distinguishing between exudative and transudative processes relies on Light’s criteria, which analyze pleural fluid protein and LDH levels relative to serum concentrations. While transudative effusions typically resolve by treating systemic conditions like heart failure, exudative cases—particularly parapneumonic effusions—require targeted interventions ranging from antibiotics to surgical drainage. Advanced refractory cases may benefit from pleurodesis to obliterate the pleural space and prevent recurrence. Supplemental biochemical markers, including cholesterol levels, further refine diagnostic accuracy when Light’s criteria prove inconclusive.
