Splenomegaly serves as a critical clinical indicator for a vast range of congestive, infectious, hematologic, neoplastic, inflammatory, and infiltrative disorders. The spleen’s dual compartments—red pulp for hematologic filtration and white pulp for immunologic response—dictate its primary functions, including the storage of one-third of the body's platelets. Clinical assessment relies on identifying symptoms like early satiety and left upper quadrant pain, alongside physical maneuvers such as percussing Traube's space or eliciting Castel’s sign. Massive splenomegaly, defined radiologically as a length exceeding 20 centimeters, often points toward specific conditions like myelofibrosis or Gaucher's disease. Management focuses on treating the underlying etiology, utilizing options ranging from JAK inhibitors like Ruxolitinib—which clinical trials show can reduce spleen volume by 35%—to surgical splenectomy, though the latter carries significant risks of infection from encapsulated organisms.
