Epoetin alfa, an erythropoiesis-stimulating agent, functions by stimulating bone marrow to produce red blood cells, effectively increasing hemoglobin and hematocrit levels in patients with chronic kidney disease or chemotherapy-induced anemia. Clinical administration requires extreme caution due to a boxed warning for serious cardiovascular risks, including myocardial infarction, stroke, and venous thromboembolism. Providers must target the lowest effective dose, typically avoiding hemoglobin levels above 11 g/dL. Successful therapy necessitates adequate iron stores, as well as sufficient B12 and folate levels, to prevent treatment failure. Monitoring protocols involve tracking hemoglobin and hematocrit, with dose adjustments of 25% triggered by suboptimal response or rapid increases. Furthermore, Epoetin alfa can artificially lower A1C readings in diabetic patients, potentially masking glycemic control issues, and requires careful management alongside medications that increase blood pressure or clotting risk.
