Empagliflozin, marketed as Jardiance, is an SGLT2 inhibitor that manages type 2 diabetes by blocking glucose reabsorption in the kidneys, effectively flushing excess sugar out through the urine. Beyond glycemic control, this once-daily oral medication offers significant cardiovascular and renal protective benefits, including reduced hospitalizations for heart failure patients regardless of diabetes status. However, its efficacy in lowering A1c diminishes when renal function falls below an eGFR of 45. The drug's mechanism introduces specific clinical risks, most notably genitourinary fungal infections and UTIs due to increased urinary glucose, alongside potential dehydration and hypotension. While empagliflozin avoids major CYP enzyme interactions, clinicians must monitor for additive effects when combined with diuretics, insulin, or blood pressure medications to prevent hypoglycemia or severe volume depletion.
