Primarily indicated for anemia secondary to chemotherapy or chronic kidney disease, this medication requires judicious dosing to avoid exceeding 11 grams per deciliter, as elevated hemoglobin levels significantly increase the risk of cardiovascular events, including myocardial infarction, stroke, and venous thromboembolism. Effective therapy necessitates adequate iron stores, B12, and folate, as deficiencies in these cofactors often cause treatment non-response. Clinicians must monitor hemoglobin closely, adjusting doses by 25% based on the rate of change, while remaining aware that epoetin alfa can artificially lower A1C readings in diabetic patients. Because the drug lacks significant traditional drug interactions, clinical management focuses on mitigating additive risks from agents that increase blood pressure or clotting potential.
