Midodrine, an alpha-1 adrenergic agonist, functions as a vasopressor by inducing vasoconstriction to manage symptomatic orthostatic hypotension. Before initiating therapy, clinicians must evaluate a patient's current medication list to identify and address potential contributors like antihypertensives, Parkinson’s medications, or PDE5 inhibitors. While effective for conditions such as cirrhosis or dialysis-related hypotension, the drug carries significant risks, including hypertension, bradycardia, and urinary retention—the latter being particularly problematic for males with benign prostatic hyperplasia. Because Midodrine is a pro-drug with an active metabolite primarily eliminated by the kidneys, dosing requires careful adjustment in patients with a GFR below 30 mL/min. Monitoring blood pressure and heart rate remains essential to balance therapeutic benefits against cardiovascular risks and avoid the prescribing cascade.
