Wernicke encephalopathy represents a critical, often under-recognized complication of thiamine deficiency, particularly within intensive care units. Thiamine serves as an essential cofactor for the citric acid cycle, and its absence triggers severe metabolic consequences, including lactic acidosis and neurotoxicity. While the classic clinical triad of encephalopathy, ophthalmoplegia, and ataxia is rarely fully present, clinicians must maintain a high index of suspicion for patients with risk factors such as malnutrition, alcoholism, or prolonged physiological stress. Diagnostic imaging, including MRI, lacks high sensitivity, making this primarily a clinical diagnosis of exclusion. Standard management requires aggressive, high-dose intravenous thiamine replacement to prevent permanent neurological damage like Korsakoff psychosis. Given the low cost and high potential for preventing nosocomial delirium, routine prophylactic thiamine supplementation for critically ill patients warrants broader implementation, especially when managing patients with high metabolic demands or those receiving diuretics.
