Altered mental status requires a systematic evaluation of potential mimickers, the necessity for immediate empiric treatment, and the patient's baseline cognitive state. Hypoglycemia, a critical metabolic cause, must be confirmed using Whipple’s triad—symptoms compatible with low glucose, documented low blood sugar, and symptom resolution upon glucose administration. Clinically, hypoglycemia is bifurcated into insulin-mediated and non-insulin-mediated etiologies. Insulin-mediated cases typically present with abrupt, prominent symptoms and an absence of ketones, whereas non-insulin-mediated causes often involve ketosis and occur secondary to systemic conditions like sepsis, renal dysfunction, or reduced nutritional intake. Distinguishing between these mechanisms is vital, as insulin-mediated hypoglycemia often points toward exogenous insulin use or insulinoma, while non-insulin-mediated processes are frequently complications of broader, multi-systemic disease states.
